I further understand that this information is being provided for consideration of financial aid; that the Tulsa Soccer Foundation may request verification of this information; and that deliberately providing inaccurate information may result in the forfeiture of the financial aid but I also understand that will not release my financial liability to the club/team.*

Once you submit this form the application process is NOT completed! You also need to scan and email documentation of the CURRENT YEAR Lunch Assistance or Food Stamp qualification. You may email to tschurricane@me.com or mail to: Tulsa Soccer Foundation, PO Box 243, Jenks OK 74037. You will receive an email regarding the status of your application once all required material is received. Please allow 30-60 days for processing.
